The Advance Care Planning lead for Somerset is Amy Giles: Amy.Giles@somersetft.nhs.uk. Amy is part of the Somerset End of Life Care Education team along with Laura James and Karen Peters.

Advance care planning is a session included in the Communication in end of life care, the Train the Champion- The deteriorating resident (for care home staff) and other end of life care training and education opportunities throughout the year.
