Advance Statement about your wishes

An Advance Statement is an Advance Care Plan where a person can record their preferences, wishes and beliefs about future care in writing. This is not a legally binding document and is different from an Advance Decision to Refuse Treatment, but can help families and professionals supporting the person know what they would like to happen in the future. In Somerset Marie Curie companions are supporting people to complete an Advance care plan which can be uploaded to the Somerset Integrated Digital e-Record (SIDeR) a shared system for Health and Social Care staff.
