The Digital Team at Somerset CCG has a mantra of “Clinically led, Digitally enabled”. They became interested in the concept of a system wide Electronic Palliative Care Coordination System (EPaCCS) to improve on the existing system, as well as improving care coordination for patients with life limiting conditions. It presented an opportunity to work across all health care providers in Somerset in an early attempt at federated working. This 30 minute video explains more.
