Marie Curie Talking about Advance Care Planning in Somerset


In Somerset we can refer people to record their Advance Care Planning discussions via the Marie Curie ‘talk about’ project. This means a Marie Curie trained companion will arrange to meet the person virtually or face to face.

An advance care plan will be completed documenting the persons preferences, wishes and hopes for the future. Once this has been agreed by the person this will be uploaded to the Somerset Integrated Digital e-Record (SIDeR).

Health and Social Care professionals in Somerset can view the advance care plan in SIDeR should they become involved in the persons care at a time when the person is unable to communicate their preferences and wishes.
